Position Overview: Spectrum Healthcare Resources is seeking a dedicated and skilled Social Worker - Case Manager to join our team at Elmendorf Air Force Base. The ideal candidate will have a Master's degree in Social Work (MSW) from a School of Social Work accredited by the Council on Social Work Education (CSWE) and hold certification as a Certified Advanced Social Work Case Manager (C-ASWCM) by the National Association of Social Workers. Alternatively, a minimum of three (3) years’ experience within the preceding five (5) years as a social work case manager in a hospital-based setting performing medical crisis counseling and discharge planning is required.
Qualifications:
- Master's degree in Social Work (MSW) from a CSWE-accredited School of Social Work.
- Certification as a Certified Advanced Social Work Case Manager (C-ASWCM) by the National Association of Social Workers OR a minimum of three (3) years' experience within the preceding five (5) years as a social work case manager in a hospital-based setting performing medical crisis counseling and discharge planning.
- At least 12 months of recent experience, within the last 24 months, providing evidence-based individual counseling in a mental health or health care facility.
- Minimum of 12 months of direct hands-on experience, within the last 24 months, with computer operations, basic word processing, data entry, and use of an automated medical record system.
- Prefer candidates with recent experience (within the past three years) providing integrated behavioral health services within a primary care setting.
- Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW).
Key Responsibilities:
- Participate in Utilization Management/Case Management meetings to review and identify opportunities for improving treatment and recommend corrective actions.
- Provide monthly in-service training for non-healthcare-practitioner members of the clinical and administrative staff on relevant social work and case management topics.
- Facilitate patient referrals to ensure comprehensive care.
- Collaborate with MTF RN Case Manager(s) in developing and implementing the Case Management Program (CMP).
- Develop local strategies leveraging nursing and social work strengths to achieve effective case management.
