Education Requirements:
- Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), National League for Nursing Accrediting Commission (NLNAC), the Commission
Skills and Qualifications:
- Strong organizational, problem-solving, and communication skills
- Ability to articulate medical requirements to patients, families/caregivers, medical and nonmedical staff in a professional and courteous manner
- Six years of broad-based clinical nursing experience
- Three years of progressively increasing managed care responsibilities with a focus on Disease Management highly recommended
- Experience with disease management, clinical practice guidelines, performance improvement, and personnel management strongly preferred
- Full-time employment in a nursing field for at least 12 of the last 18 months is mandatory
- Possess a thorough clinical knowledge and breadth of experience to contribute to population-level assessment and management
- Ability to identify appropriate use of Clinical Practice Guidelines (CPGs), conduct patient self-care education, and track patient outcomes
- Extensive knowledge of Disease Management components including outcomes measurement, collaborative practice, CPGs, patient self-management, population identification processes, and process improvement
Job Description: As a Registered Nurse specializing in Disease Management at Travis Air Force Base, you will play a pivotal role in improving patient outcomes and promoting wellness within our community. Your responsibilities will include but are not limited to:
- Collaborating with interdisciplinary teams to develop and implement disease management strategies
- Conducting patient assessments and developing individualized care plans based on clinical guidelines
