RN - Case Manager

Spectrum Healthcare Resources has a potential opportunity for a civilian Full-time Case Manager RN at Robins Air Force Base, located near Warner Robins, GA.

Position Highlights:

• Emphasis on work-life balance 
• Outpatient environment & updated facilities (not a remote position)
• No weekend, holiday, or on-call responsibilities 
• Monday-Friday

Responsibilities:

Assessment: Identification of patients for case management; comprehensive collection of patient information and medical status; and continue evaluation of an established plan of care.

• Planning: Collaboration with the patient, family/caregiver, primary care giver, primary provider and other members of the health care team for developing an effective plan of care.

• Facilitation: Care coordination and communication among all involved parties.

• Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received. Preferred:

• Knowledge or experience in occupation health • Familiarity with Department of Labor (DLA) and Occupational health & Safety Administration guidance, (workers compensation law)

Requirements:

• Bachelor of Science in Nursing from accredited institution
• Current and valid RN state license
• A required minimum of 3 years for ADN or 2 years for BSN full-time experience in clinical/occupational case management working with adults. Must have the knowledge and skills to effectively apply the following core case management functions
