Spectrum Healthcare Resources has a potential need for a Registered Nurse – Case Manager at David Grant Medical Center, Travis Air Force Base, CA. The Nurse will provide case management services within the Integrated Disability Evaluation System. This is a full-time, on-site position, working Monday through Friday, 7:30am-4:30pm.
Requirements
- Associate Degree in Nursing from an accredited program.
- Current, full, active, and unrestricted RN license.
- At least 3 years of experience within the last 4 years working as a registered nurse case manager.
- Current Basic Life Support (BLS) certification.
- Possess one of the following certifications:
- Certified Case Manager (CCM)
- Certified Disability Management Specialist (CDMS)
- Certified Rehabilitation Registered Nurse (CRRN)
- Certified Occupational Health Nurse (COHN) or COHN-S
- Advanced Certification in Continuity of Care (ACCC)
- Certified Rehabilitation Counselor (CRC)
- Nurse Case Manager (RN-NCM)
- Care Manager Certified (CMC)
Duties
- Participate in all phases of the Case Management Program (CMP) to ensure established CM standards of care are met.
- Provide nursing expertise in assessment, planning, implementation, coordination, and monitoring of case management processes.
- Develop treatment plans that ensure continuity of care and optimal wellness outcomes.
- Liaise with community agencies and organizations to ensure seamless care transitions.
- Educate patients and caregivers on their care plans.
- Facilitate interdisciplinary team meetings and participate in discharge planning.
- Ensure accurate data collection and documentation.
- Conduct peer reviews and quality reviews as required.
- Travel outside the local MTF may be necessary (reimbursable as applicable).
- Strong organizational, communication, and problem-solving skills required.
- Proficiency with Microsoft Office applications and familiarity with military healthcare systems.
