Spectrum Healthcare Resources is seeking a compassionate and experienced Nurse Case Manager (Purchased/Referred Care – PRC) to support the Indian Health Service (IHS) at the Fort Peck Service Unit in Wolf Point, Montana.
In this role, you’ll be a trusted advocate for patients—helping ensure they receive timely, coordinated, and appropriate care while navigating the PRC referral process. Monday–Friday | 8:00 a.m.–4:30 p.m. | Federal Holidays Off

Opportunity to use your clinical judgment and case management expertise
Slower pace than acute care with meaningful patient relationships
Coordinate and manage care for patients with complex or special healthcare needs
Review outpatient referrals for medical necessity, appropriateness, and cost-effectiveness
Work closely with PRC staff, providers, and outside facilities to ensure smooth transitions of care
Track consults and referrals, including deferred services, to support quality improvement efforts
Serve as a clinical resource to improve patient outcomes and streamline referral processes
Graduate of an accredited ADN or BSN nursing program
Active, unrestricted RN license (any U.S. state or territory)
3+ years of RN experience, including 2+ years in case management or PRC-related work
BLS certification (American Heart Association)
U.S. Citizenship required
