Registered Nurse – Case Management, Outpatient

Spectrum Healthcare Resources has a potential opportunity for an experienced Outpatient RN Case Manager to join the healthcare team at Joint Base Elmendorf-Richardson, Alaska.

Use your clinical knowledge and care coordination skills to help military patients navigate their healthcare needs.

Position Highlights

• Full-time opportunity

• Monday–Friday, 7:30 AM–4:30 PM

• Medical, dental, vision, and 401(k) benefits

Position Requirements

• U.S. Citizenship

• Associate Degree in Nursing or higher from an ACEN- or CCNE-accredited program

• Current, unrestricted RN license from any U.S. jurisdiction

• 3 years of RN Case Management experience within the last 4 years

• Case Management certification (CCM, CDMS, CRRN)

• Current BLS certification

Key Responsibilities

• Coordinate outpatient treatment plans and specialty referrals

• Assess patient needs and develop individualized care plans

• Collaborate with providers and community resources

• Educate patients and coordinate ongoing care
