This role requires a compassionate, organized, and skilled professional ready to make a positive impact.   Qualifications: - Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), National League for Nursing Accrediting Commission (NLNAC), the Commission on Collegiate Nursing Education (CCNE) or as otherwise identified. - Twelve (12) months of recent full-time experience in nursing/health care management for adults, children, families, seniors, or groups is required. - Must possess organization, problem-solving, and communication skills to articulate medical requirements to patients, families/caregivers, medical, and non-medical staff in a professional and courteous way. - Skillful and tactful communication with individuals who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, or occasionally dangerous. - Knowledgeable in medical privacy and confidentiality (HIPAA), accreditation standards of Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC). - Proficient in computer applications/software including Microsoft Office programs, MS Outlook (e-mail), and internet familiarity. Responsibilities: - Assessment: Identify patients for case management, collect comprehensive patient information and medical status, and evaluate established plans of care. - Planning: Collaborate with patients, families/caregivers, primary providers, and healthcare teams to develop effective care plans. - Facilitation: Coordinate care and communication among all involved parties. - Advocacy: Provide support for patients and families/caregivers to ensure timely and appropriate care is received. Additional Skills: - Thorough clinical knowledge and experience to contribute to population-level assessment and management. - Identify appropriate Case Management candidates and conduct patient self-care education. - Track patient outcomes and demonstrate expertise in Case Management components such as outcomes measurement, collaborative practice, and process improvement. Be a crucial support in fostering resilience and transformation.   These services will be in support of the United States (U.S.) Department of Labor, Office of Workers’ Compensation (OWCP) and the Division of Federal Employees’ Compensation (DFEC). RCs will be expected to manage injured workers cases in person and telephonically in their assigned area.    Position Details: - Provide vocational counseling and guidance to individuals disabled under FECA.
