Position: RN Case Manager
Requirements:

Education: Shall be a graduate from an associate (ADN) or baccalaureate degree (BSN) program in nursing accredited by a national nursing accrediting agency recognized by the US Department of Education.

License: Maintain an active, valid, current, and unrestricted license (with no limitations, stipulations, or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction.

Experience: A minimum of 3 years for ADN or 2 years for BSN full-time experience in clinical case management working with adults, children, families, seniors, and groups.

Certification: Certified Case Manager by Commission for Case Management (CCM) or American Nurses Credentialing Center (ANCC) -OR- certification eligible with 3 years of experience for ADN or 2 years' experience for BSN in full-time clinical case management with children, families, seniors, or groups.
Responsibilities:
- Provide comprehensive and compassionate case management services to patients.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Advocate for patients' needs and coordinate healthcare services.
- Conduct assessments, evaluations, and follow-ups to ensure quality care.
- Maintain accurate and timely documentation of patient care activities.

RN Job Duties:

RN Job Requirements:
- Graduate from a National League of Nursing accredited school (Diploma, AD, or BSN)
- RN licensure in any of the 50 states, active and unrestricted
- Minimum 1 year experience in Occupational Health or related area such as public health/community health, outpatient clinic, school nursing, or physician office nursing
- Minimum 3 years RN experience preferred
- Recent clinical experience required
- AHA (American Heart Association) Basic Life Support Certification, CPR/AED
- Must be willing to travel to more than one clinic within a day, no more than

Team Health and Spectrum Healthcare Resources has an excellent opportunity for a Licensed Professional Nurse (LPN) in Chillicothe, Ohio. The LPN will be providing Correctional Medical care for those housed at at the Ross County Jail.
Position Highlights:

Job Duties:
- Provide a range of medical care to include primary, addiction, urgent and behavioral health care

- Graduate of a school of practical or vocational nursing approved by a state agency or accredited by the ACEN
- A minimum of 12 months of experience in a primary care setting within the last 24 months
- Active, unrestricted OH LPN license
- BLS from the American Heart Association
