Spectrum Healthcare Resources has a potential, full-time opportunity for a civilian Registered Nurse - Case Manager to work full-time at Los Angeles Air Force Base in Los Angeles, Ca.   You will be working with active service members, their dependents and Tri-Care Insured patients.   Position Details:Ind.Nurse - Assessment: Proactively identifies and evaluates patients and families for case management from a variety of sources such as direct referrals, the Medical Evaluation Board (MEB) process, the healthcare system involving transitions of care, and facility staff. Conducts systematic, on- going, thorough collection of patient’s physical, emotional, psychological, social and medical status and information via direct patient contact and other relevant sources such as professional and non-professional caregivers, medical records, and family/caregiver interviews - Planning: Develops an appropriate patient-specific plan of care to include short and long- term goals, objectives and actions. Coordinates, collaborates, and obtains approval of the plan among the patient, family/caregiver, primary provider and other members of the healthcare team. - Implementation: Guides the patient and family/care giver through the healthcare system, maximizing use of resources. Coordinates and executes the plan of care, optimizing access to appropriate services. Ensures necessary referrals are ordered by the appropriate discipline and coordinated. Serves as an advocate for, and ensures education is provided to, the patient and family/caregiver as required. Promotes adherence to treatment plans for improved healthcare outcomes. - Coordination: Ensures coordination of care delivery processes, to include alternate healthcare settings and the home environment, for the purposes of enhancing the patient's health and wellness, safety, productivity, and quality of life, and for providing the most beneficial, cost- effective health care. Ensures timely and appropriate provision of services. - Complex Care Coordination for Active Duty and Veterans: May serve as case manager for wounded, ill, and injured Active-Duty Service Members and Veterans requiring complex care coordination. Will include the family/caregiver in support of the recovering service member/veteran. Will also function as a member of the care management team including the Recovery Care Coordinator (RCC), Lead Coordinator, Physical Evaluation Board Liaison Officer (PEBLO), Wounded Warrior, and VA Services. - Monitoring: Documents and updates the treatment plan as needed in accordance with existing guidelines/instructions of Department of Defense (DoD), Military Branch (Air Force/Army/Navy), and DHA/Military Medical Treatment Facilities (MTFs). Conducts and/or participates in program evaluation as directed. • Ensures compliance with standards of care and practice (The Joint Commission (TJC), Accreditation Association Ambulatory Health Care (AAAHC) & Case Management Society of America (CMSA)). • Excellent patient assessment skills. Possesses excellent oral communication skills for patient education and telephone management of patients. Possesses knowledge of professional care theories, principles, practices, and procedures to perform patient assessments with specific experience in the case management evaluation and assessment of pediatric, adult and geriatric patient population. • Demonstrates sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to a variety of patients and healthcare staff from diverse backgrounds. • Demonstrates ability to prioritize, organize, problem-solve and coordinate daily activities to support effective improvement in individual patients. • Facilitates using Utilization Management (UM) data to identify high-risk/high-cost patients within MTF population, factoring Adjusted Clinical Groups (ACGs) and Illness Burden Index (IBI). Using individual patient care data to engage Case Management interventions needed to facilitate patient gaining optimal access to quality healthcare services based upon clinical evaluation of patient health care, psychosocial/ wellbeing and emotional/social and socio-economic needs.  Position Qualifications: - Education:  Registered Nurses (RNs) shall have successfully completed a Bachelor of Science Degree in Nursing (BSN) from a National League of Nursing (NLN) or Commission on Collegiate Nursing Education (CCNE) accredited college or university. Licensed Clinical Social Workers (LCSWs) shall have successfully completed a Master of Social Work (MSW) degree program accredited by the Council on Social Work Education (CSWE). Active state or compact RN license to work in the assigned areas - Experience:  RNs must have at least three (3) years full-time experience of documented clinical experience (inpatient/outpatient) with at least one (1) year experience of case management and/or care coordination. LCSW must have a minimum of two (2) years full-time experience in professional case management or discharge-transitional planning programs for adults, children, families, seniors and groups. Previous inpatient/outpatient experience at Military Medical Treatment Facilities, Veteran’s Affairs (VA), home-health care, skilled nursing facility, hospice or mental health services is highly desired. Experience with Medicare Eligible patients is preferred. - Board Certification: Certified (or certification-eligible) Case Manager by Commission for Case Management Certification (CCMC) or America Case Manager Association (ACMA) or Certified Social Work Case Manager (C-SWCM) through National Association of Social Workers (NASW) is desired. Certified CMs/SWs will provide a copy of the certifications prior to performance on this contract. - Licensure/Registration: RNs must possess a current, active, full and unrestricted license in any U.S. State or jurisdiction, be able to practice using a nurse compact state license or Enhanced Nurse Licensure Compact (eNLC) or have a license from any United States jurisdiction. LCSWs must possess a current valid license having no adverse action pending from any state or national licensing/certification agency. A Social Worker license is defined as a license that allows the Social Worker to work independently without clinical supervision. Graduate from a college or university accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) or completion of a Didactic Program in Dietetics (DPD) accredited by the ACEND, or completion of a post- Baccalaureate degree Dietetic Internship Program of supervised practice (for a minimum of 1200 hours) accredited by ACEND.
